Intake Form - The Lantern TL Coaching

Thank you for connecting with me to co-create on your health and wellness journey! It takes a spirit of bravery and courage to be willing to look within, connect with others, and be open with all that life has to offer!


Who I am + What I do

My name is Twyla and I am a Holistic Nurse Health and Wellness Coach. Think of me as a blend of a Registered Nurse, a guide, and a co-creator in your wellness. Someone who looks at the whole picture of your health, this means mind-body-spirit-energy and environment, not just one piece of it. I help people feel clearer in their minds, rooted in their bodies, balanced emotionally, and guided by their own inner wisdom for health and wellbeing.


How we Work Together

In our sessions, we discover your innate ability to heal, and I believe you are the master of your own health, wellbeing and inner joy; I am simply a guide to this discovery. I use mind-body-spiritual-energetic-environmental tools – like guided imagery, mindfulness, breathwork, grounding practices, reflection, energy work, lifestyle nursing, positive psychology, high performance strategies, movement, intuition, and artistic expression– to help you connect to deeper realms of self, others, and beyond the self to live a more purpose-filled life.


A Partnership, Not a Prescription

My role isn’t to tell you what to do – it’s to awaken the power inside of you. To empower, encourage, inspire and support in creative ways that resonate with your soul. Together, we’ll uncover what feels aligned, realistic, and supportive for you, by learning to tune into your body-mind-spirit-energy-environment while creating balance and harmony in these areas. I help you find clarity, build confidence, and create changes that become a part of you, because they’re rooted in your values, your rhythm, and your real life.


An Invitation

If you’re soul is tired, confused, or vacant… If the vibrance has drained from your life… and you are craving a space where you feel heard, understood, supported… a sacred space to breathe again… If you want to explore new possibilities for your health, energy, or overall wellbeing… to live a life of balance, harmony, ease and tranquility…  I’d love to connect, and feel entirely blessed to walk along this journey with you.

Agreements:

Safety and Confidentiality – This is a safe space, one that I want you to feel comfortable sharing and opening up. To create this space I ensure that information shared with me will be kept confidential (other than Rose, my academic advisor who will be reviewing content for educational purposes).

Compassion, Kindness, & Respect – I will do my part to treat you with compassion, kindness, and respect, treating these meetings as a sacred space for us to co-create together

Presence - I ask that we minimize disruptions to the best of our ability, ie) silencing our phones, removing distractions, so we can honor and value our time spent together. Also, you may notice me typing or looking at notes, this is simply to help me keep organized and track some of our conversations and key themes that may be arises so I can best guide you and our interactions.

Flexibility – In saying that, I also want to honor flexibility in our partnership, so if there are requests to allow you to engage such as kids or animals in the background, having to pause for a bio-break or tend to these needs please do not hesitate to let me know.

Openness &  Willingness -  I ask that you participate as fully as you can in these sessions and be courageous in perhaps trying new things or becoming curious about different perspectives.

Allowing for Rest - If at any point the sessions become uncomfortable or too overwhelming for you, please ask for a pause or redirection. This also incorporates Trauma-Informed Care, a lens that I am particularly sensitive to and honor in our journey together. Please let me know if you become overstimulated or overwhelmed and need a moment.
